Please send the completed order form by postal mail to Amphora at the above address. Please enclose a cheque for payment in full. Cheques should be made payable to ‘Veitch Moir Ltd’. With prior agreement, we can accept payment on delivery. If payment on delivery has been agreed, you may email the completed form as an attachment to AmphoraUK@aol.com or fax it to 01403-791977.
